4.3 The information you provide in your intake form is stored in the clinic management software Simple Clinic. Personal information is stored in secure online services. If you completed this form online prior to your initial consultation, please refer to Simple Clinic's privacy policy.
Cloud Storage
5.1 Information you share via email or text message with us may be stored securely, with access protected by a security code. With your consent during the consultation, Patrick Thomas Naturopathy may take photographs of your supplements, medications, or test results for research purposes or to track treatment progress. These images are temporarily stored and then moved to your encrypted file within 24 hours of the consultation before being deleted from the phone. Photos taken on mobile devices are not uploaded to any cloud storage.
5.2 We may use a service called Mailgun to send electronic newsletters to you via email. If you subscribe to this list, your name and email address are stored on Mailgun servers to facilitate communication. You can opt out at any time by clicking the 'unsubscribe' button at the bottom of emails. Please refer to Mailgun's privacy policy for further information.
5.3.1 Patrick Thomas Naturopathy uses a clinic management software called Simple Clinic.

5.3.2 Simple Clinic is a cloud-based web application that stores your name, email address, phone number, and other details provided in the intake form if you book online, as well as case notes and health information, including sensitive information. Please refer to Simple Clinic's privacy policy for further information.
5.4 Third-party testing services, such as Naturopathic Services, may require information such as your name, date of birth, gender, and presenting symptoms. These services comply with the Australian Privacy Policy as stated on their websites.
Sensitive information
6.1 Patrick Thomas Naturopathy understands that some personal information is particularly sensitive.
6.2 We will only collect sensitive information through secure methods, including:
Through your intake form completed online during the booking process or sent via email ahead of your first consultation
Via email if you send us test results or other information relevant to your health history
During a telephone, face-to-face, or online consultation
6.3 The sensitive information we may ask you to provide may include:
Your birth date, medical history, family history, allergies and intolerances, current medications, supplements, and diagnosed health conditions to provide you with a safe and effective naturopathic service.
Information about the food and fluids you consume, dietary and lifestyle preferences or habits to understand factors influencing your health and provide appropriate recommendations.
Information about your physical well-being, such as blood pressure, pulse rate, temperature, weight, height, nail, tongue, or iris data, to inform our naturopathic understanding of your case and treatment plan.
We may discuss your emotional well-being during consultations and record some of the emotions you're experiencing to track your progress and identify appropriate treatment options.
Others' access to your information
7.1 Only Patrick Thomas Naturopathy has access to sensitive material disclosed during consultations.
7.2 Employees of Australian Health Products Pty Ltd, trading as Go Vita Tuggerah, and agents for Patrick Thomas Naturopathy may require access to your medication schedule for the purposes of filling Naturopathic Prescriptions in accordance with relevant regulations.
7.3 Personal information such as name, address, date of birth, phone number, and/or email may be accessed by employees of Australian Health Products Pty Ltd, trading as Go Vita Tuggerah, and agents for Patrick Thomas Naturopathy for creating, updating, and confirming consultation appointments, mailing products and correspondence, and invoicing.
Your access to your personal information
8.1 You have the right to request details of the personal information we hold about you in accordance with the provisions of the Privacy Act 1988 (Cth). Please email us at [email protected] if you would like a copy of the information or believe that any information we hold about you is inaccurate, out of date, incomplete, irrelevant, or misleading.
8.2 We reserve the right to refuse to provide you with information that we hold about you in certain circumstances set out in the Privacy Act.
